Eugene Tetsuto Fukumoto, 77 years old, Amache, Colorado born,  resident of Glendale who passed away on August 7, 2021 at his residence.

He owned Health Claim Services, was an author of books regarding Healthcare and workers' comp, and had a passion for deep sea fishing.

He is survived by brother, Arnold Koji Fukumoto of WA; sister, Gail Miyako Shoffner; nephew, Jason Sei Brown; also survived by aunt, and many cousins here and in MN, WA, FL, CO, HI and in Japan.

The family will have a private memorial service at a later date.
